Free POLST & LIFE Form Packets

Hours: Monday - Friday, 9:30 AM - 4 PM
Cost: Free for all Orcas Island residents

LIFE Form (Lifesaving Information For Emergencies)

Critical information our EMTs need:

How to Use These Forms

Complete the forms with current, accurate information

  • LIFE form: In the provided envelope in a visible location

  • Best location: On or in your refrigerator/freezer (where EMTs are trained to look)
